Global Xchange Cards - Ordering and Order

If you have reached this screen by purchasing a
card from our shopping cart, your purchase transaction was successful!
IMPORTANT:
If you have
just purchased a Global Xchange card, you still need to send in proof of
identification documentation before a card can be sent out to you. Follow the
instructions below to submit this information to us so we can finish processing
your card order.
To get a card, we
require from you the following three things faxed or scanned and e-mailed to us:

a LEGIBLE copy
of a government issued photo identification card (front and back) or
passport

a LEGIBLE copy
of any bill in your name where the address matches your photo ID card's
address

Completion and submission
of the following signature form

Please note that we will not accept ID document packages in excess of 10
megabytes. Any submissions made to us in excess of this file size will be
deleted upon receipt.

Existing Cardholders and Outstanding Card Orders
For
more information on our card products and recommendations/advise on how to use
your Global Xchange card, please go to
www.globalxchange.biz/cardprograms.htm .
Any Global Xchange customer who has an existing
Global Xchange card or has an outstanding card order may obtain one of our
cards by sending us the above identification information. As soon as we obtain
this information from you, we will get your card on its way to you. No
additional payment from you is required to get one of the new cards.
If
you are ordering a second card or additional cards from us, you do not need to
resend us your proof of identification information unless your address has
changed since you ordered your first card. If you have moved, please resend us
identification information showing your new address and also be sure to update
your address information in both the "Profile" section of your Global Xchange
account and in the "Profile" section of the organization you belong to that uses
the Global Xchange system. Do this to ensure that your card gets sent to the
right address.
Lastly,
if you have paid for a card for someone other than yourself and they have sent
in their identification information, you need to let us know as we do not send
out cards until we are able to match the card payment with the identification
submitted. If this has been done, please send us an e-mail to
sales@globalxchange.biz that
contains the date and payment information along with who the card was for so we
can match the payment with proof of identification received.
